SafeSearch for Google and YouTube not working

Recently, I enforced YouTube restricted mode, and Google SafeSearch and it just stopped working one day. NxFilter still shows the setting is applied but it is not working anymore. Any ideas on what could be the issue?

If I do 'dig www.google.com' while Safe-Search is on,

You see that I get forcesafesearch.google.com? Test it with dig or nslookup.
